Optimal time for insertion is during menstruation. Alternatively, the IUD may be introduced w/in 5 days of unprotected coitus or w/in 15 min of delivery of the placenta or abortion in the 1st trimenon. Replace IUD after 5 yr for Mona Lisa Cu375/Mona Lisa NT Cu380/Mona Lisa NT Cu380 Mini, & 10 yr for Mona Lisa CuT 380A.
Contraindications
Absolute: Malignant diseases of the genital tract, vag bleeding, pregnancy, history of ectopic pregnancy or predisposing factors, genital tract infection, STD during the last 12 mth (except bacterial vaginitis, repeated herpes infection, hepatitis B), abortion w/ infection during the last 3 mth, pelvic inflammatory disease (PID), uterine malformations (congenital or acquired), allergy to copper. Relative: Valvular heart disease, anaemia, coagulation disorders, anti-inflammatory treatment, Wilson's disease, multiple sexual partners, nulliparity.
Conduct a medical history & examine pelvic & abdominal cavity & cervical smear prior to insertion. Determine position of the uterus & the size of the uterine cavity to ensure correct insertion. Re-examined after 1 mth to determine whether IUD is properly placed & for signs of infections. Vag discharge, pelvic pain and fever. Excessive bleeding or dysmenorrhea. Perforation of the uterus during insertion. Do not perform diathermy (short wave & microwave) of the sacral or abdominal region.
